Patient Privacy Consent

At Foothills Family Optical, great care has been taken to ensure the privacy and confidentiality of your personal information and health records. Nevertheless, in an effort to serve you better, it may become necessary to share some of your information with other stakeholders including but not limited to private health care providers, ophthalmic laboratories and our practice management software provider . Your consent states that we have informed you of some of these entities, to give you an idea of how your information is being used and to obtain your permission to share your information when necessary

i) Foothills Family Optical may be required to share personal health information and records with private health care providers, ophthalmic labs and some other third parties only on an “as needed” basis or as required by law;

ii) Information may be stored locally on a server or in a secure location on-site.

iii) A verbal and/or written permission for customers to be personally contacted by Foothills Family Optical or on behalf thereof by a third party in order to receive appointment reminders, order confirmations and status updates;

iv) Written permission to be contacted personally by Foothills Family Optical or on behalf thereof by a third party in order to receive information regarding offers, promotions and exclusive sales tailored specifically to meet my optical needs.

v) Personal information will not be shared with third parties for marketing or promotional purposes.


Remakes / Returns / Warranty Information

Every pair of glasses ordered from Foothills Family Optical is custom made to order. Therefore, we are not able to refund any products that are not resaleable. We are happy to service all of our products and back them up with a one-year warranty for quality and workmanship from the manufacturer. Warranties cover against defects and do not cover accidental breakage, abuse, or loss. A one-time accidental breakage discount of 50% will apply within the first year for replacement of frames and lenses. Original frame may not be available.


Doctor Changes / Non-Adapts

Foothills Family Optical will honor a one-time Rx change made by the Doctor for 60 days following the original order. 

In the event that a patient is not satisfied with the lens or frame choice, a one-time exchange will be provided at no charge within 60 days.


Patient’s Own Frame

Although we use the utmost care when handling your frame, there are many factors that go into why a frame may break (quality, age, material, pre-existing damage, etc). Occasionally a frame will break in the process of adjusting or while manufacturing new lenses. Because we do not have control over these factors, Foothills Family Optical cannot be held responsible for any breakage / damage to a patient’s own frame.


SMS/Contact Information Disclosure

By providing a telephone number, email address, and/or other forms of contact information, clients/customers/patients are giving verbal and/or written consent to be contacted by SMS text message, phone call, and/or email from Foothills Family Optical for order updates; questions regarding an order; or other information pertaining to the business.
